What is 8D?
Eight Disciplines

8D is a method for solving a problem as a team through a structured, eight-step approach. It became widespread in automotive, and customers often require an 8D report from suppliers.
How does it work?
- D1 Form the team · D2 Describe the problem · D3 Contain it.
- D4 Find the root cause (occurrence and escape) · D5 Choose and verify the permanent corrective action.
- D6 Implement and validate effectiveness · D7 Prevent recurrence (systemic) · D8 Recognise the team and close.
- Some implementations add a D0 planning step at the start.
Often confused with
The 8D form and the 8D process
The 8D form reports results. The process is executing the steps with owners, deadlines and evidence; filling in the form is not the same as running it.
Common mistakes
- Skipping D3 containment and moving to root cause without protecting the customer.
- Finding only the occurrence cause in D4 and skipping the escape cause (why it was not caught).
- Treating implementation as effectiveness verification in D6.
